Key Responsibilities
• Provide intensive support to young people with SEND.
• Develop personalised action plans focusing on strengths, needs, and aspirations.
• Support access to volunteering, work experience, and educational opportunities.
• Act as the primary point of contact for young people and their parents/carers.
• Coordinate with a team of professionals to deliver holistic support.
• Attend and contribute to multi-agency meetings.
• Ensure high attendance rates and timely completion of assessments and action plans.
• Engage in 1-to-1 activities tailored to young people's needs, including vocational, PSHE, and enrichment activities.
• Assist in exploring next steps beyond the programme, ensuring smooth transitions.
• Maintain accurate records and ensure compliance with safeguarding and data protection protocols.

As part of this commitment anyone working in a post with children or vulnerable groups will be expected to undertake an Enhanced DBS check with/without barred list checks.
As this post involves working within one of these groups, we will require a Disclosure & Barring Service check and/or subscription to the DBS online service. The suitability of all prospective employees or volunteers will be assessed during the recruitment process in line with this commitment.
